Rotator cuff tear- best rotator cuff repair gives best recovery.

The rotator cuff is a group of muscles and tendons that surround the shoulder joint, keeping the head of the upper arm bone firmly within the shallow socket of the shoulder.

Rotator cuff tears affect 30–50% of persons over the age of 50 years and are a common cause of function-limiting pain and weakness of the shoulder. Rotator cuff tear commonly occurs following a fall on the shoulder. At old age sometimes degenerative tear can happen without any injury.

Who are at risk of rotator cuff tear?

  • People over 50 are at greater risk- as normal wear and tear of cuff muscles goes along with ageing
  • Athletes and players indulged in overhead sports like- tennis, baseball etc.
  • People who do repetitive lifting or overhead activities are also at risk for rotator cuff tears

The most common symptoms of a rotator cuff tear include:

Rest pain and night pain is the most frequent complaint in partial cuff tear. One may feel pain while lifting and lowering the arm from a lifted position. Its often difficult to lie down on the affected shoulder. When tear is big or complete one may feel weakness while lifting or rotating the arm.

At first, the pain may be mild and present only when lifting your arm over your head, such as reaching into a cupboard. Over time, when tear increase in size and torn cuff retracts away from the shoulder the pain may become more noticeable at rest and it may become very difficult to do routine activities, such as combing your hair or reaching behind your back.

How diagnosis is made: After clinical examination MRI is done. It is the gold standard to make a diagnosis. MRI also gives a fair idea about repairability of the cuff apart from  confirming the diagnosis.

Why early repair is necessary?

With time protein of torn muscles get replaced by fat and muscles become very weak. Also torn muscles get retracted inside away from the shoulder joint so with time repair become very difficult and early repair of the rotator cuff tear gives best results.

Studies have proven that early repair of a traumatic rotator cuff tear provides better results in terms of shoulder function in comparison with a delayed repair.

Which repair gives best strength and early recovery?

My preferred technique is arthroscopic double-row repair. Single-row repair was the initial technique described for rotator cuff tendon-bone reattachment, but because of the limited ability to restore the anatomic footprint, double-row repair configurations were developed. In double-row repair, bridge-type constructs link both medial and lateral row anchors and this increases tendon-bone compression, stiffness of the construct, and increases load to failure. When compared to single-row repairs, double-row repair typically provides a higher rate of healing.
